Queens Facilitated Enrollment Project

     The Queens Facilitated Enrollment Project at Flushing site promotes, recruits, and provides enrollment assistance to qualified New York State residents to apply for government funded health insurance programs.  These programs include: Child Health Plus, Family Health Plus and Medicaid.  This project is funded by the New York State Department of Health and led by Safe Space.

     The goals of the Facilitated Enrollment project are to provide free or low cost affordable health insurance and to increase accessibility to health care.  All New York State residents, under the age of 65, may be eligible to sign up for these health insurance plans.  Benefits include physician service, well visit, immunization, emergency room visit, in-patient care, diagnostic & laboratory tests, eye care, dental care, mental health service and other related medical services.


     Child Health Plus is a program for children under the age of 19.  It consists of part A and part B.  The eligibility for each part is based on income and residency status.  There may be a low cost premium required if the income is above the limit.

     Family Health Plus is a program for adults 19 to 64 years of age.  Eligibility is based on income and residency status.

     Medicaid is a program for adults 19 to 64 years of age.  Eligibility is based on income, resources and residency status.


     The application procedure is simple and free of charge.  Chinese/Korean bilingual speaking staff is available to assist in the application process.  The application must be done in person at Charles B.
