Social Work Department:

Social Work Services

Social work service is available for all Charles B. Wang Community Health Center patients. Our social workers work closely with your doctors and nurses as a team to provide comprehensive care and services for you and your family.

Our Commitment:

We are committed to working with you to improve your health and quality of life.

Our Services:

Social Work Case Management Service:
We coordinate your care and service.
Our social workers will help you communicate with your doctors and external medical or social service providers to make sure you receive the proper care or service.

Individual and Family Supportive Counseling:
Sometimes you may feel stressed due to various life circumstances and would like to speak with someone.
Our individual and family supportive counseling enhances your ability to cope with your problems (e.g. stress from current medical condition, relationship problems).

Apply for Public Insurance and Government's Benefits:
Your access to health care is our top priority!
If you are uninsured or underinsured, we can provide necessary assistance to help you obtain government medical insurance if you are eligible. Assistance with applying for other government benefits is also available for our patients as needed.

Advocacy:
Your service needs are to be met and should not be limited due to the language, physical, cognitive, or other limitations.
Our social workers will advocate on your behalf to ensure proper treatment and services are delivered and received if appropriate.

Linkage to Service or Resources:
Referral service is available for our patients who have financial, homecare, housing, developmental, educational, emotional, and other social service needs.

Crisis Intervention:
A crisis can be a stressful life experience.
Social work crisis intervention is available for patients who are confronted with crisis-inducing events such as domestic violence but cannot resolve the issue on their own.

Contact Us:

If you need social work service, let your doctor know or give us a call. We would be happy to speak with you to provide services that meet your needs.
