The Health Center's social workers provide valuable links between the patient, the clinical team, and community resources. The social service team's bilingual and bicultural services include collaborating with the Health Center's clinical team to develop and implement biopsychosocial treatment plans, advocating to ensure that patients receive appropriate treatment and entitlements, managing cases and facilitating services, and providing health education, supportive counseling, and crisis intervention. Social services are available at ALL Center locations. |
