September 11 Fund/Chinatown Health Partnership:

     Following the September 11 tragedy, the Charles B. Wang Community Health Center in collaboraton with the September 11th Fund and the Lutheran Family Health Center, initiated the Chinatown Health Partnership.  The goal of the Partnership is to provide health care services to individuals and families affected financially by September 11.  Program participants are eligible for up to 12 months of low cost medical, dental, and other specialty services at the Health Center.

     The September 11th Fund is a joint venture between the New York Community Trust and the United Way to help respond to the immediate and long-term needs of victims, their families and communities affected by the September 11 attack.  The Fund extended program benefits for an additional six months to members who continue to qualify for services.  If the patients currently enrolled are unemployed or earning less than 70% of their pre-September 11 wages, and do not have any other health insurance, they may qualify for an additional six months of coverage.  These patients must complete an extension of benefits form to receive these additional services.

     The enrollment deadline for new applicants is January 31, 2004.  Eligibility specialists are available from Monday to Friday, 9 AM to 5 PM.  Call the Hotline at (212) 226-8339 to make an appointment or come to 125 Walker Street, Second Floor and ask for assistance from the 9/11 Eligibility Specialist.
