The Charles B. Wang Community Health Center is offering the COVID-19 Vaccine to interested individuals based on the New York State guidelines.

All individuals 5 years of age and older that reside in the United States are eligible to receive the vaccine. We currently offer:
- Pfizer vaccine for dose #2, dose #3 and booster for 12 years of age and older
- Pfizer vaccine for dose #2, dose #3 for 5 through 11 years of age
- Moderna vaccine for dose #2, dose #3 and booster for 18 years of age and older
所有在美國居住的5歲及以上的人士都可以接種新冠疫苗。我們目前提供：

CDC now recommends receipt of a booster dose at least five month after completing the Pfizer or Moderna primary series. Individuals received the Janssen (Johnson & Johnson) vaccine as primary series are eligible for a booster dose at least 2 months after the 1st dose.
目前，疾病控制與預防中心 (CDC) 建議已經接種了兩針輝瑞或者莫德納疫苗的人士，可在接種了第二針的至少5個月後接種加強針。已經接種了強生新冠疫苗的人士，可在接種了第一針的至少2個月後接種加強針。
To receive COVID-19 vaccines at the Charles B. Wang Community Health Center,
- Minors 16 or 17 years of age, parental or legal guardian consent should be provided in person, by phone, or in writing, at the time of vaccine appointment.
- Minors who are 5 through 15 years of age, in addition to consent for vaccination, parent/guardian or an adult caregiver designated by parent/guardian should accompany the minor. The parent/guardian must still provide consent to the vaccination.
在王嘉廉社區醫療中心接種新冠疫苗，
- 16、17歲的青少年：在注射新冠疫苗的當天，父母或監護人可親自陪同，也可通過書面或電話的形式給予同意。
- 5至15歲的青少年：需由父母或監護人，或由父母或監護人指定的成年人陪同下接種疫苗。父母或監護人仍必須給予同意。
