As a team-based medical home, we provide additional services to make sure that you and your family receive comprehensive and well-coordinated care. Women, Infants, and Children (WIC) program which provides supplemental foods, health care referrals, and nutrition education for low-income pregnant women and mothers with newborns, and for infants and children up to age five who are found to be at nutritional risk. Health Insurance If you do not have health insurance, we have health insurance enrollment counselors available to help you enroll under the New York State of Health or government benefits, including Medicaid, Child Health Plus & Qualified Health Plan. By appointment only. Nutrition counseling by certified dieticians. By appointment only. Language access line at all sites to help translate for patients who speak languages that our staff does not speak. Patient portal will give you online access to easily communicate with your provider, refill your medication, request appointments, request referrals, request test results, send a message to your provider and view your medical summary. |
