Appointment FAQs

How can I make an appointment?

It is best to call for an appointment so that we can plan for your visit and maximize your convenience. Please visit Our Services page to find the service(s) you need and call the location most convenient for you.

What should I prepare for my first visit?

For your first visit, you should prepare to:

  • Arrive about 30 minutes before your appointment so you can complete registration forms.

  • Bring a photo ID and insurance card if you have one. If you do not have health insurance, we will work with you to see if you qualify for health insurance or our sliding fee scale.

What emergency and urgent care services do you provide?

We offer urgent care services for when you are sick. You may also call us for after-hours "urgent medical advice." In the case of a medical emergency, please call 911.

Where are your health centers located?

We have health centers located in Flushing, Queens and Lower Manhattan, all of which are near major forms of public transportation like the subway or bus. For more information, please visit our Locations page.

Is there somewhere I can access my lab tests or send a message to my provider online?

Once you are enrolled as a patient, you can sign up for the Health Center’s Patient Portal, a secure web-platform where you can access your lab tests, ask for prescription refills, or send a message to your provider with questions. You can also request an appointment by sending us a message through the Patient Portal.

Please speak to your receptionist to sign up for the Patient Portal. You will need an email address to set up your portal account.

What is the 340B Prescription Discount Program?

The Health Center’s 340B Prescription Discount Program is for everyone and there is no cost to join. It provides great savings for people without health insurance. You can participate in the 340B Prescription Discount Program by choosing one of the following partner pharmacies as your preferred pharmacy and filling a prescription by a CBWCHC provider. Patients reserve the right to choose their own pharmacy without joining the 340B Prescription Discount Program.

What if I don’t have insurance?

We provide care to everyone regardless of your ability to pay or your health insurance status. If you qualify, we can help you enroll in government sponsored health insurance plans, including Medicaid, Child Health Plus, and Qualified Health Plan. The sliding fee scale discount program is also available to those who qualify.

Need assistance with health insurance? Our Health Insurance Enrollment team can help! You can call (212) 226-8339 (Manhattan), (718) 886-7355 (Flushing Queens), or (718) 298-5000 (Jamaica, Queens).

What is the sliding fee discount program?

The sliding fee discount program is available to all patients, regardless of your health insurance status. If you are uninsured or underinsured, we can work with you to see if you qualify based on your household income and family size. We will need financial information to determine your eligibility.

What forms of payment do you accept?

We accept all major credit cards, checks, or cash for services.
