New Patient Visit
Your first visit with us to establish care. Please allow extra time for this appointment.
Annual Wellness Visit
A preventative visit focused on your overall health, screenings, and health planning.
Follow Up Visit
A visit to review an existing condition, recent test results, or treatment plan for ongoing care with your provider.
Urgent Care Visit
A visit for urgent, but non-life threatening concerns that need same day or next day attention.

Pediatrics
New Patient Visit
Your child’s first visit with us to establish care. Please allow extra time for this appointment.
Annual Wellness Visit
A preventative visit for patients ages 4 – 21 focused on their overall health, screenings, and health planning
Follow Up Visit
A visit to review an existing condition, recent test results, or treatment plan for ongoing care with your child's provider.
Urgent Care Visit
A visit for urgent but non-life-threatening concerns that need same-day or next-day attention.
Well Child Exam Visit
A comprehensive preventative checkup for patients ages 0 – 3. This visit includes tracking growth and development, immunizations, conducting health screenings, and providing parental guidance.

Obstetrics & Gynecology (Coming Soon)
New Patient Visit
Your first visit with us to establish gynecological care. This visit may include breast and cervical cancer screenings, STD testing, contraceptive counseling, and management and menopause care.
Annual Wellness Visit
A yearly preventative gynecological exam for current patients. This visit includes routine screenings and wellness care. Note: Annual exams must be scheduled at least 12 months after the previous annual exam.
Follow Up/Urgent Care Visit
For current patients who need ongoing care, review of test results, treatment management, or evaluation of new or existing symptoms and concerns.
Pregnancy-related Visit
For prenatal care, pregnancy confirmation, or other pregnancy-related concerns.
