MyChart Patient Portal Login

On, Tuesday, September 16, 2025, the Charles B. Wang Community Health Center will be moving to a new and secure patient portal powered by MyChart. This new portal will provide you with 24/7 access to your health information, tracking of your care and progress, requests for appointments or medication refills, and much more!

Effective September 16, 2025, your old login information will no longer be active from the ezAccess patient portal. All medical records will be transferred except for your inbox messages. You can still download any existing messages, if needed, up until Friday, September 12.

**You will receive information on how to sign up via email and/or text after September 16, 2025 or when you arrive for your next appointment.**

MyChart Patient Portal

Charles B. Wang Community Health Center’s electronic health records system or “patient portal” helps improve our patients’ experience. Patients have better access to their care and providers through an easy-to-use online patient portal service called MyChart.

Through MyChart you can:

  • Communicate with your CBWCHC care team
  • See your medication list
  • Get lab results, vaccination records, and other health records
  • Make a payment
  • Request an appointment
  • Have a smoother telehealth visit experience

To request a copy of your medical records through MyChart

  • Navigate to My Record > Document Center and select: "View, Download, or Send Visit Records" or "Request Additional Records"

Frequently Asked Questions about MyChart

If you cannot remember your password, your password no longer works, or you have too many unsuccessful login attempts, you will need to reset your password. Click "Forgot Password?" and follow the steps to reset your password. If you continue to have issues, please call our Health Education team at (212) 966-0461 to speak to a staff member about getting your password reset.

Your activation code has probably expired. Activation codes are only good for 30 days. If your code has expired, please call our Health Education team at (212) 966-0461 or visit the Health Center to start the process to get a new one. You can get your new activation code via text, email, in the mail, or in person at a clinic. New activation codes cannot be given over a phone call.

MyChart is a secure means for patients to access parts of their health records through a web browser and cellphone apps. ezAccess and OCHIN are hosts of the health records system. By switching to OCHIN from ezAccess, CBWCHC patients will receive enhanced services for an exceptional patient experience.

If you have access to your family member’s medical records, you can view most of the information in their records in the same way that you view your own. Learn more about accessing family member medical records with this manual.

MyChart displays information from the day of enrollment forward. If you enrolled in MyChart before your appointment, your provider should be on your list of provider options in MyChart. If you enroll in MyChart after your appointment, your provider will be added to your list of care team members after your next appointment.

CBWCHC MyChart is not able to send messages to specific medical assistants, dental hygienists, front desk staff, eligibility specialists, interpreters, providers filling in from another location, and other non-medical/dental support staff.

You will generally receive an answer within three (3) business days. Please note that MyChart should not be used for urgent situations. If you need immediate care or have life-threatening symptoms, please call 911.

CBWCHC will have limited ability for patients to schedule appointments with providers. If you are unable to schedule directly or need an appointment as a new patient, you can request an appointment through MyChart and someone will respond to your request. You can also call to set up an appointment.

Your lab results may not be ready yet or are still being reviewed by your care team. Most test results are released to MyChart the same day that they are marked as finalized unless considered sensitive by NY law.

If it has been longer than 72 hours and you have not heard from your provider, please send a message to your provider or call your CBWCHC front desk to learn more. Lab results cannot be shared over the phone unless you speak to a registered nurse, physician assistant, or physician.

Charles B. Wang Community Health Center (CBWCHC) partners with an organization called OCHIN for MyChart support. If you have an account with another organization that is within the OCHIN collaborative, no action is needed. You will be able to see your information from all organizations within OCHIN. If you have an account with another organization that is outside of the OCHIN collaborative, you will need to link your accounts together to see everything in one place. This feature is called Happy Together in MyChart.

How to link accounts:

  • Step 1 – Log into your CBWCHC MyChart account.
  • Step 2 – Click Menu and then click Link My Accounts
  • Step 3 – Use the Search bar that instructs you to “Discover accounts to link” and type in the organization’s name. Please follow the prompts to link your records.

Charles B. Wang Community Health Center (CBWCHC) partners with an organization called OCHIN for MyChart support. On the MyChart app, you will need to search for Charles B. Wang Community Health Center as the organization.

How to setup Charles B. Wang Community Health Center MyChart in the app:

  • Step 1 – Open the MyChart app on your iPhone or Android device.
  • Step 2 – You will be presented with a login screen for whichever organization you had previously signed into. At the top of the screen, select the “Switch Organizations” button.
  • Step 3 – Select the “Add Organization” button.
  • Step 4 – Search for “CBWCHC” and select "Charles B. Wang Community Health Center" that appears as a search result.
  • Step 5 - When you return to the login screen, the Charles B. Wang Community Health Center banner will display across the top. You can now use your CBWCHC login information to access MyChart through the MyChart app.

You can update your email address and contact information by logging on and clicking Menu > Account Settings > Personal Information > Edit. If you need to update your username, please call the Health Education team at (212) 966-0461.
